Forms

Many of the forms are in Adobe Acrobat Reader format.  If you don't have an Adobe Acrobat Reader, visit http://www.adobe.com/products/acrobat/readstep2.html to obtain a reader.

Many of the forms are large.  Expect long download times.  The sizes are given with a conservative estimated download time at 56Kbit/sec

PDF forms will probably open in your browser and your browser may indicated "Done" when, in fact, download is in progress.
PDF Forms can be printed and filled out by hand, or filled in and printed while online.  Filled in forms cannot be E-mailed or Saved.  Sent the printed results by US Mail.

Working Dog Certificate Application

 Download into your browser the Working Dog Degree Application ( fill it out, print, and mail it to the indicated Working Dog Chair with the appropriate fee payable to the I.W.P.A.   Working Dog Degree Application in rich text format will work in most any word processor.  Download to your disk and you can fill it out and email to the Working Dog Chair (Donna M Libbey) AND follow-up with a check, payable to I.W.P.A., that you send via US mail (the form could also be sent via US mail).  Suggestion: place your word processor in "over-write" mode to fill out the rich text form.

Download to your disk the PDF Sanctioning Packet Zip Folder OR the Word/Excel Sanctioning Packet Zip Folder (Word files are actually RTF files usable by most any word processor) in order to obtain all the forms needed for reporting pull results.  There is an RTF file in each packet named Instructions on Completing Pull Forms.

PDF Forms

You can print the forms and fill out by hand, or fill them out on you computer and print the completed form. 

The completed forms then can be mailed to the Sanctioning Chair.

Word / Excel Forms

You can print the forms and fill out by hand, then mail to the Sanction Chair

OR

Fill them out on your computer, then save and/or Print.  If you saved a copy, you can E-Mail them to the Sanctioning Chair & to the Statistician.  If you printed, you can mail them to the Sanctioning Chair.

Hint: if you E-mail, package them in a zip file.
