Membership Application

Working Dog Certificate Application

Download into your browser the Working Dog Degree Application (fill it out, print, and mail it to the indicated Working Dog Chair with the appropriate fee payable to the I.W.P.A.). Working Dog Degree Application in doc format or the Working Dog Degree Application in Adobe Acrobat PDF Format will work in most any word processor. (If you don't have an Adobe Acrobat Reader, visit Adobe to obtain a reader.) Download to your disk and you can fill it out and email to the Working Dog Chair (Donna M Libbey) AND follow-up with a check, payable to I.W.P.A., that you send via US mail (the form could also be sent via US mail).

You can print the forms and fill out by hand, then mail to the Sanction Chair

OR

Fill them out on your computer, then save and/or Print. If you saved a copy, you can E-Mail them to the Sanctioning Chair & to the Statistician. If you printed, you can mail them to the Sanctioning Chair.

Hint: if you E-mail, package them in a zip file.
